ADR-002: Single managed entrypoint

Decision: NETLOAD only PRecAnnot.Plant3D2026.dll. Status: accepted. Why: removes orchestration of 20 independent loads and centralizes commands/Ribbon. Consequence: internal dependencies must be packaged as CLR dependencies, not NETLOAD targets.
